The ongoing opioid crisis in the United States, worsened by the prevalence of fentanyl, has resulted in an increased number of overdose-related deaths, disproportionately affecting the African American population and exposing vulnerabilities among the Soma li immigrants. Although there has been a growth in the overdose prevention initiatives, there have been unresolving disparities because of structural, cultural, and systemic barriers. This article uses a qualitative phenomenological research design to investigate the experiences of these groups in Minnesota. The instrument used to gather data was semistructured interviews, which were conducted with 14 participants, comprising African American and Somali adults, and analyzed with the support of the thematic analysis based on Critical Race Theory and the Social Determinants of Health framework. According to the findings, medical mistrust, fear of institutional systems, and stigma are deeply rooted hinders overdose prevention, which encourages silence and delays in seeking help. There are structural issues like financial lack, inaccessibility due to lack of transport and fragmented healthcare systems that deter access to services. Moreover, the lack of culturally responsive care, especially the language barrier and cultural knowledge deprivation, is a great obstacle to the engagement of the Somali participants. These issues are interconnected, which enhances the unequal results of prevention. In general, the paper shows that interrelated social and structural circumstances influence the risk of overdose and not personal behavior. To overcome such obstacles, culturally responsive, community-based, and equity-related interventions are needed, which will help to build trust, increase access, and decrease any systemic differences in healthcare delivery.
