Despite major progress in the knowledge and management of PIRDs, several problems persist. In order to enable prompt and reliable identification of affected patients, future research should concentrate on enhancing early diagnosis by the integration of nextgeneration sequencing, functional immunological assays, and new biomarkers.[1,3] A deeper comprehension of genotype-phenotype relationships will enhance disease prognostication and assist customized treatment plans. It is anticipated that as precision medicines continue to advance, PIRD therapy options will increase. Personalized combination therapy, pathway-specific small-molecule inhibitors, and emerging biologics may minimize the long-term toxicity of traditional immunosuppression while increasing efficacy. Furthermore, it is hoped that improvements in HSCT, such as better donor screening and optimal conditioning regimens, may increase survival and immune reconstitution in patients with severe illness.[3,4] Gene therapy and genome-editing techniques, especially those based on CRISPR-Cas9, present encouraging opportunities for fixing mutations that cause disease and attaining longterm remission or cure. These approaches are a significant step toward individualized therapy in PIRDs, even if they are still the subject of clinical research. Additionally, multicenter clinical trials, long-term follow-up studies, and international collaborative registries are crucial for bolstering the body of data, establishing uniform treatment guidelines, and enhancing patient outcomes globally.[3,4,5] Even with significant advancements in PIRD research and therapy, there are still a number of unanswered questions. Biologics, inhibitors targeted at pathways and combinations of therapies will allow for more effective treatment and reduction of possible toxic effects of standard immunosuppressive therapy.
