At the same time, the evidence base remains limited by the short duration of follow-up, small sample sizes, lack of standardization of EAT assessment, and the scarcity of interventional studies in patients at the highest cardiovascular risk, as well as by the absence of long-term data on the impact, of EAT reduction on hard clinical endpoints [7,10,12,13,20,21,22].

The use of non-probability sampling may limit generalizability and introduce selection bias. Retrieving the participants food history via the FFQ, could introduce recall bias as some of the participants might face difficulties providing accurate reflections of their meals. Additionally, there is the possibility that members of the case cohort have had prior exposure to diet therapies which could influence both their current meal intake and responses to present a “healthier” intake. These biases could be addressed in further studies through the use of alternative dietary intake assessment methods that are less subjective.

The crudeness and imprecision of the NOVA classification system may limit accurate assessment of ultra-processed food-related health outcomes. - The heterogeneity in dietary intake across countries may limit the applicability of evidence from different contexts to dietary guidelines targeting a specific context. - The paper highlights the need for high-quality, context-specific research to inform recommendations, implying that current evidence may be limited.
