What the literature leaves open

  • When utilizing CW-NIRS, applying a single set of scat- tering coefficients across individuals does not account for 1 3European Journal of Applied Physiology scattering changes due to individual variation in muscle quality and make-up (Ferreira et al.

  • Whether different target sites respond optimally to different parameters, and whether these relationships hold in cognitively impaired populations with altered mitochondrial dynamics, remain open questions. Advancing toward a precision-medicine intervention will require further study of key light parameters, particularly pulsation frequency and its interaction with other dosing variables.

  • • The findings support further investigation of an integrated scoring system based on cerebral and splanchnic flow, systemic flow measures such as SVCF and/or cardiac output, and oxygenation parameters to identify infants most likely to benefit from transfusion.

Integrated cerebro-splanchnic blood flow and regional oxygenation monitoring in transfused anemic preterm infants · 2026 · DOI
  • Long-term follow-up for neurodevelopmental outcomes was not available. The relatively small sample size and lack of a control group make it difficult to distinguish transfusion effects from other physiological changes over time. Neonatal echocardiography and Doppler imaging were performed by a single operator who was not blinded to transfusion status, although the recorded images were reviewed by a consultant radiologist and a consultant neonatologist who were unaware of transfusion status. Celiac artery flow rather than superior mesenteric artery flow was measured. Although the superior mesenteric artery is a key supplier to the midgut, the celiac artery was selected because it was more accessible for consistent, high-quality insonation in very small preterm infants with abdominal distension or gas, and because celiac artery flow has been used as an indicator of overall splanchnic perfusion changes.

Integrated cerebro-splanchnic blood flow and regional oxygenation monitoring in transfused anemic preterm infants · 2026 · DOI
  • Background Despite the growing interest in functional near-infrared spectroscopy (fNIRS) for practical brain-computer interface (BCI) applications, prefrontal hemodynamic responses during rapid serial visual presentation (RSVP) tasks remain poorly characterized, even though these tasks demand sustained attentional engagement under fast-paced visual streams.

Prefrontal fNIRS hemodynamic correlates of attentional load during rapid serial visual presentation tasks · 2026 · DOI
  • h t m l practices, and open challenges. However, the current feature space is limited to the hori- zontal direction xabs, depth zabs, and radius rabs, with the fixed vertical direction yabs, absorption coefficient µa, and the assumption of a single spherical absorber in the simulation box. Our current neural network model is specifically designed for brain tissue optical properties and is therefore limited to applications involving brain structures.

  • Absorbing solitary pulmonary nodule phantoms with improved optical similarity to malignant tissue (absorption and scattering properties matching actual tumors in the NIR range) have not been developed. Preparation of such phantoms is necessary for accurately interpreting absorption–scattering contrast mechanisms specific to malignant lesions under DTNI illumination.

Near-infrared diffuse transmittance imaging for localization of pulmonary nodules · 2026 · DOI
  • The bronchial wall causes approximately threefold broadening of the illumination spot, reducing spatial resolution and directional intensity. Optical modeling under anatomically realistic conditions—particularly for the 20 mm tissue thickness range typical of bronchoscopic access—is required to quantify achievable spatial resolution and inform dedicated pleuroscope optics design.

Near-infrared diffuse transmittance imaging for localization of pulmonary nodules · 2026 · DOI
  • Light delivery optimization has only evaluated uniform illumination geometry; alternative diffusing fiber tips and modified illumination geometries have not been experimentally tested for enhancing internal illumination uniformity and efficiency in bronchial tissue. Systematic comparison of light delivery configurations is needed before pleuroscope integration.

Near-infrared diffuse transmittance imaging for localization of pulmonary nodules · 2026 · DOI
  • Interlobular septa consistently produced high-intensity structures that distorted the apparent nodule outline in DTNI images. The mechanisms underlying differential light transmission between septa and parenchyma must be modeled and experimentally characterized to improve nodule boundary delineation in heterogeneous lung tissue.

Near-infrared diffuse transmittance imaging for localization of pulmonary nodules · 2026 · DOI
  • The relationship between solitary pulmonary nodule depth relative to the bronchial wall and DTNI signal detectability has not been characterized experimentally. Systematic measurements across lesion depths are required to establish clinical detection limits and provide data necessary for clinical feasibility assessment.

Near-infrared diffuse transmittance imaging for localization of pulmonary nodules · 2026 · DOI
  • Tissue optical clearing via passive immersion with VG/PG mixtures enhanced septal signal but failed to uniformly clear functional parenchyma. Active delivery methods for optical clearing agents—including perfusion, aerosolization, and controlled inhalation protocols—require systematic evaluation to achieve spatially uniform clearing suitable for in vivo bronchoscopic DTNI application.

Near-infrared diffuse transmittance imaging for localization of pulmonary nodules · 2026 · DOI
  • Iron oxide phantom formulations failed to generate adequate absorption-based contrast for nodule localization in DTNI imaging. Future work must systematically optimize hematite particle concentration, particle size distribution, and wavelength-dependent optical properties to achieve absorption contrast comparable to malignant tissue in the 850 nm NIR window.

  • Although there are still many problems with this technique, such as incomplete knowledge of how light propagates through the head, NIRS will not only open a window on brain physiology for subjects who have rarely been examined until now, but also provide a new direction for functional mapping studies.
