Identification of Coagulation Phenotypes Using Viscoelastic and Conventional Assays During Veno‐Venous Extracorporeal Membrane Oxygenation: A Hierarchical Clustering Analysis · 2026 · DOIThis study had several limitations. First, this was a sin- gle-centre retrospective cohort study, and the number of patients was small; therefore, multivariate analysis was not performed. Consequently, the results might have limited generalizability. Second, ECG parameters might be useful only in patients with a sinus rhythm. In the present study, all patients had sinus rhythm; however, the use of P-wave parameters in patients with atrial fibrillation rhythm was not evaluated. Third, although we defined the deteriorated group as patients who died within 30 days of Impella implanta- tion, the longest duration of acute-phase mortality in this study was 15 days. Furthermore, in cases of death occurring at a later stage, the proportion attributable to non-cardiac causes is likely to increase; therefore, it is unclear whether the definition of “within 30 days” is entirely accurate, and we believe there are some limitations to the generalizability of these findings.
Predictors of the need for escalation therapy in patients with cardiogenic shock treated with an Impella device: a novel approach using electrocardiography · 2026 · DOIECPella (ECMO + Impella) is increasingly used for cardio- genic shock from cancer-therapy–related cardiac dysfunc- tion. Newer fully magnetically levitated pumps reduce shear stress and lower rates of stroke, thrombosis, and GI bleeding. These advances are especially important for cancer patients, who already face high infection and bleeding risks. Progress in this field depends on close collaboration between oncol- ogy and heart-failure team, along with research focused on better anticoagulation strategies and patient selection to break down the traditional contraindication of active cancer for VAD support [13].
