What the literature leaves open
Through historical records, the world has witnessed devastating outcomes of nuclear wars hitherto, most especially their inimical effects on human health. Moreover, while recent technological advancements continue to unleash sophisticated instruments of mass destruction, global health experts and international bodies must become more intentional and proactive on the possible global health impacts of unleashing such instruments on the environment, humans, and agriculture. A more realistic and prag- matic approach could be to ensure countries, especially those possessing nuclear weapons, renew their commitment to nuclear non-proliferation treaties and global dis- armament efforts. Recent international initiatives further reinforce these efforts, includ- ing the United Nations General Assembly Panel of Scientific Experts on the Effects of Nuclear War and the Treaty on the Prohibition of Nuclear Weapons, both of which underscore the catastrophic humanitarian and global health consequences of nuclear weapons and the urgency of prevention [16, 17]. This commitment will ensure the reduc- tion of nuclear arsenals. The commitment should include transparent verification mech- anisms to ensure compliance. Nonetheless, in the case where absolute commitment cannot be ascertained among countries with heavy nuclear weapons, the World Health Organization (WHO) and national health agencies must show proactivity by developing contingency plans to curtail the possible environment psychological, and health impacts Okeke et al. Discover Public Health (2026) 23:882 Page 4 of 5 of nuclear war. This may include the establishment of rapid-response emergency teams, radiation treatment facilities, and stockpiles of essential medicines and supplies. Specific measures embedded within national security doctrines should also be framed as components of global health security. Policies such as no-first-use commitments, de-alerting nuclear arsenals, and reducing unilateral launch authority can be viewed as upstream preventive strategies that reduce the risk of catastrophic health emergencies. These measures align with current global health security priorities, including preven- tion, preparedness, health-system resilience, emergency response capacity, and multilat- eral cooperation. Furthermore, research and public education should be integrated into nuclear disaster management and control. The interdisciplinary research should focus on the long-term health implications of nuclear war on human health, including genetic mutations, cancer prevalence, mental health disorders, and transgenerational impacts. The outcomes of the research should serve as the foundation for public education. In addition, educational institutions should assist in adopting and integrating nuclear risk awareness in curricula and emergency preparedness training to equip health professionals and communities with practical survival and response skills.
Recent confrontations along the India-China border (Doklam 2017, Galwan Valley 2020) have remained limited to conventional engagements, but the paper does not analyze the mechanisms or thresholds that prevent escalation to nuclear confrontation.
Indian policymakers should realize that firstly, the operationalization of the CSD crosses Pakistan’s nuclear threshold and it requires Islamabad to unleash strategic and tactical nuclear weapons (TNWs); secondly, the Pak-India crisis and deterrence stability is functional due to the sense of mutual vulnerability; thirdly, the credibility of nuclear deterrence has not been tested and fourthly, the deterrence stability solidified the crisis and strategic stability.
” This debate persists, in part, because existing studies suffer from a key limitation: they are not designed to infer a causal connection between NPT membership and nuclear proliferation.

India's nuclear deterrence strategy must reckon with challenges from both Pakistan and an expanding Chinese military power, but the paper does not analyze how India can simultaneously maintain credible deterrence against two nuclear adversaries.
The paper discusses how China's Belt and Road Initiative and geopolitical expansion indirectly impact the nuclear balance, but does not provide empirical evidence or modeling of these indirect effects.
The India-China nuclear relationship is described as generally more stable than India-Pakistan dynamics, but the paper does not systematically analyze the causal factors explaining this difference or provide comparative analysis.
While the paper notes that nuclear deterrence has allowed a delicate balance in which crises must be carefully managed, it does not provide a framework or methodology for analyzing what constitutes 'careful management' or how to measure its effectiveness.
Recent scholarship has inadequately examined the unintentional consequences of the paradox in conflicts below the nuclear threshold, particularly those relating to the potential for aggression to escalate uncontrollably.

This study examines why off-site nuclear accident planning has been a low federal priority, why the problem is intergovernmentally complex, and why this sub-policy issue remains controversial.
The effectiveness of the Non-Proliferation Treaty has remained limited to the major alliance systems of the two superpowers, and its future now depends on the reconciliation of sharply differing views concerning why and how the spread of nuclear weapons should be prevented.
