  • The present study has several limitations inherent to the available literature on spinal angiolipoma. First, the evi- dence base is dominated by case reports and small case series, making the pooled dataset vulnerable to publica- tion bias, selective reporting, and inconsistent documenta- tion of clinical course, imaging findings, operative details, and follow-up. Second, outcome definitions were not uni- form across studies; terms such as recovery, improvement, and stable deficit were variably reported, which may have affected outcome classification. Third, follow-up duration was heterogeneous and sometimes limited, likely leading to underascertainment of late recurrence. This issue is par- ticularly relevant given the very low number of recurrence events, which resulted in sparse-event data, wide confidence intervals, and inability to perform robust adjusted model- ing of recurrence risk. Finally, although exploratory pooled comparisons suggested an association between extent of resection and favorable outcome, the case-based nature of the data precludes causal inference and these findings should be interpreted as hypothesis-generating rather than definitive. Prospective multicenter registries using stan- dardized radiological, surgical, pathological, and outcome definitions, together with uniform long-term follow-up, are needed to refine risk stratification, clarify indications for instrumentation, and better define recurrence patterns and surveillance strategies.
