What the literature leaves open
While prior work has demonstrated that food allergy can be modulated by gut microbes, little is known about how food allergen-specific CD4+ T cells are affected by gut microbial composition.
This review highlights growing evidence that infection-associated gut dysbiosis may contribute to long-term variability in neuroinflammatory susceptibility by influencing systemic immune homeostasis. A key mechanistic insight is that microbiota-derived metabolites, including SCFAs and tryptophan-derived indole compounds, may interact with host epigenetic machinery, thereby linking peripheral immune activity to sustained transcriptional regulation in innate immune cells. To summarize the previous sections, main findings are graded by the strength of evidence in Table 1, distinguishing four levels: human association (correlational, typically confounded by clinical care), animal causality (interventional, e.g. faecal microbiota transplantation), extrapolation from adult stroke or non-HI inflammatory models, and therapeutic speculation. From this overview, two research priorities can be defined: unaddressed mechanistic links and conceptual gaps in preclinical and clinical study designs (Table 2). At the conceptual level, two questions remain. First, sex differences are well documented separately for the developing brain, immune system and gut microbiota [86–88]. HI itself produces sex-specific patterns of lesion size, microglial activation, cell-death pathways and peripheral immune cell function [47, 89–92]. Whether sex-specific gut dysbiosis contributes to these dimorphic brain and immune responses has not been examined. Sex-stratified designs in both preclinical and clinical studies will be essential to avoid missing biologically relevant effects. Second, the developmental memory of perinatal microbial perturbations and its potential impact on lifelong immunity and neurological function remain poorly defined. Clinical and preclinical data are sparse and inconsistent. After caesarean section, overall microbiome structure normalizes as the gut matures although selected genera differ at five years. Rodent prebiotic supplementation from P5 to P14/15 produces a community that is no longer detectable in adulthood, whereas perinatal fructo-oligosaccharides in pigs persistently modulate the adult faecal microbiota at nine months. However, this founding period is fraught with “hidden gateways” to “pathology.” Mohamed et al. offer a cautionary tale about how early iatrogenic disordersespecially NSAID-induced dysbiosis can trigger autoimmune responses and worsen autism spectrum disorders (ASD). This link between microbial metabolic production and neurodevelopment is further hardened by Serrano-Tomás et al., whose meta-analysis identifies p-cresol as a tangible metabolic signature of the severity of ASD, transforming an intestinal by-product into a clinical compass. As the axis matures, the complexity of the actors involved in the brain-gut axis increases. This Research Topic enhances the mycobiome and the molecular master-regulators to the dominant bacteriocentric model. The fungal dimension was treated by Hadrich et al. and Wang et al.
