Additionally, our study is compliant with methodological guidelines including PRISMA,28 MOOSE,41 and guidelines specifically designed for conducting systematic reviews and meta- analyses in pain medicine.60 61 Several limitations should be considered when interpreting the results of this study. First, the predominance of observational studies and the substantial heterogeneity of studies lower the certainty of evidence and warrant great caution when inter- preting the results of this study. Second, some analyses were limited to only two to four studies and there were small sample sizes for some of the subgroup analyses (ie, stimulation wave- forms) which yielded wide CIs and limited interpretability. Third, the inability to quantify possible placebo effects or psychological dependence on opioids likely influences outcomes. Fourth, as noted above, some physicians and countries require intentional opioid tapering prior to SCS implantation. While there is not strong evidence to suggest this strongly influenced our results one way or another, it is possible that opioid use could be overes- timated or underestimated depending on the intentional opioid tapering circumstances. Lastly, there likely exist significant vari- ances in opioid tapering protocols and practices following SCS implantation that were not adjusted for in our analyses.

Moreover, advances in biological interfacing technologies—including optogenetics and bioelectronic medicine—offer exciting possibilities for achieving finer spatial and temporal control over neural circuits. These innovations could enable stimulation that is not only personalized but also biologically harmonious, tailoring neural modulation to the patient’s intrinsic rhythms and synaptic connectivity. Clinical consensus now strongly supports a forward-looking strategy centered on individualized therapy design. Guidelines emphasize the importance of customizing stimulation waveforms, developing multidisciplinary care teams, and maintaining structured long-term follow-up programs to ensure consistent patient benefit and safety. Collaborative clinical frameworks that integrate pain specialists, neuroscientists, engineers, and data scientists are likely to become increasingly essential for optimizing outcomes and sustaining innovation.[17] As SCS enters this next stage of development, its evolution from a static device therapy to a dynamic, networked neurotherapeutic platform will redefine standards in pain management and neurological rehabilitation.
